| Wage Tax Statement | This statement declares the wage salary for a certain year broken down by pay date, the amount withheld for wage tax, and the taxing authority to which it was paid. | PSU Form W-4 | EMPLOYEE'S WITHHOLDING ALLOWANCE CERTIFICATE (FORM W-4): This form authorizes the University to make the appropriate withholdings for both Federal Income Tax and Local Wage taxes. CERTIFICATE OF FOREIGN STATUS OF BENEFICIAL OWNER UNITED STATES TAX WITHHOLDING: A W-8BEN form needs to be completed and submitted to Payroll by non-resident or resident aliens that have money through their student account that is over and above tuition. An email letter is sent to an individual from Student Loans and Scholarships Office if they need to complete this form. The form is then returned to the Payroll Office for processing. If a person does not return the form by the date requested it could result in a 30% federal tax withholding. |

| Credit Union Deduction | CREDIT UNION DEPOSIT AUTHORIZATION and DISTRIBUTION OF PAYROLL DEDUCTION: The DEPOSIT AUTHORIZATION form is to authorize, change or cancel a payroll deduction with deposit to the Penn State Federal Credit Union. The DISTRIBUTION form is used to instruct the Penn State Federal Credit Union of the categories/amounts to which member deductions are to be applied. A Credit Union form should be completed and submitted to the Credit Union for processing. They will notify the Payroll Office of any changes. A Credit Union Distribution of Payroll Deduction should be completed to instruct the Credit Union of the categories/amounts to which member deductions are to be applied. You need to complete both forms to change or to cancel a deduction |
