Athletic Injury Report - For the reporting of athletic injuries occurring at the varsity levels only. (8/1/2006)
Affidavit of No Insurance - (8/1/2006)
Contract Remittance Cover Sheet (6/09)
Embedded Education Abroad Programs Checklist - This form is used when Program offered for academic credit. Policy FN-20
Incident Report - For reporting accidents resulting in a bodily injury or property damage only.
Indemnification Agreement - use of PSU Premises by Non-PSU
Groups.
International Group Risk Management Checklist - This form is used when Program has no academic credit is involved.
Property Loss Report - Any Department, College or Campus sustaining a loss should complete this form.
Standardized University Entertainment Agreements (Master Entertainment, Stage Sound, DJ and Speaker) - Revised 5/09 are available on the FO website.
Vehicle Accident Report - University driver must complete no matter how minor the accident.
